Add questionnaires to verification level
Extend verification levels with questionnaire steps.
To use questionnaires for verification, you need to add a questionnaire to a verification level as a separate verification step. You can add questionnaires both to standard levels (for individual and company applicants) and applicant actions.
Get started
To add a questionnaire to the verification level:
- Create a verification level as described in this article.
- On the Steps tab, under Verification steps, select Add new step, then choose the Questionnaire option.
- Select the questionnaire that you want to use for the level and save your changes.
Multiple questionnaires
In case you need to use multiple questionnaires during the applicant onboarding process, you can either configure the Workflow Builder, or add several Questionnaire steps to the same verification level as described below:
- Create a verification level.
- On the Steps tab, under Verification steps, select Add new step, then choose the Questionnaire option.
- Select the questionnaire that you want to use.
- Repeat steps 2 and 3 to add more questionnaires to the level. When selecting the Questionnaire option again, each subsequent step will be labeled as 2nd Questionnaire, 3rd Questionnaire, and 4th Questionnaire.
- Save changes.
Adding multiple questionnaires to a level works properly when you need to display several questionnaires within a single verification level without applying additional logic or conditions. For example, when all questionnaires must be completed sequentially as part of the same verification flow.
Note
You can only add up to four Questionnaire steps within a single level. Each step must contain a different questionnaire.
Use cases for multi-step questionnaires
Onboarding flows may require applicants to complete multiple steps in which they answer topic-specific questions and upload relevant documents at different stages. Below, you can see a couple of common examples.
Example A: Business verification
The applicant progresses through several questionnaires that capture different sets of business-related data and supporting documents.
Example B: Identity and Payment methods
The applicant first completes a questionnaire focused on baseline identity verification, then proceeds to a separate questionnaire dedicated to payment method verification:
- Identity and address. The applicant answers identity-related questions, and uploads the ID and proof of address documents.
- Payment methods. The applicant answers questions about card or account ownership, limits, and usage, then uploads supporting documents such as card proof, a bank letter, or an account screenshot.
Questionnaire display in the Dashboard
When multiple questionnaires are applied in a verification flow, mind the following behaviors in the Dashboard.
